Nutrition in Primary Health Care: Using a Delphi Process To Design New Interdisciplinary Services

Abstract

Purpose: A modified Delphi process was used to identify key features of interdisciplinary nutrition services, including provider roles and responsibilities for Ontario Family Health Networks (FHNs), a family physician-based type of primary care.
Methods: Twenty-three representatives from interested professional organizations, including three FHN demonstration sites, completed a modified Delphi process. Participants reviewed evidence from a systematic literature review, a patient survey, a costing analysis, and key informant interview results before undertaking the Delphi process. Statements describing various options for services were developed at an in-person meeting, which was followed by two rounds of e-mail questionnaires. Teleconference discussions were held between rounds.
Results: An interdisciplinary model with differing and complementary roles for health care providers emerged from the process. Additional key features addressing screening for nutrition problems, health promotion and disease prevention, team collaboration, planning and evaluation, administrative support, access to care, and medical directives/delegated acts were identified. Under the proposed model, the registered dietitian is the team member responsible for managing all aspects of nutrition services, from needs assessment to program delivery, as well as for supporting all providers’ nutrition services.
Conclusions: The proposed interdisciplinary nutrition services model merits evaluation of cost, effectiveness, applicability, and sustainability in team-based primary care service settings.

Résumé

Objectif: Une méthode Delphi modifiée a été utilisée pour déceler les caractéristiques des services nutritionnels interdisciplinaires, notamment les rôles et les responsabilités du fournisseur dans les réseaux de santé familiale (RSF) de l'Ontario, où des médecins de famille dispensent des soins primaires.
Méthodes: Au total, 23 représentants d'organisations professionnelles, entre autres trois sites expérimentaux de RSF, ont été soumis à la méthode Delphi modifiée. Avant d'appliquer la méthode, les participants ont examiné les travaux publiés, une enquête auprès de patients, une analyse des coûts et les résultats d'entrevues auprès d'informateurs clés. Des énoncés décrivant diverses options de services ont été élaborés lors d'une rencontre en personne suivie de deux tours de questionnaires acheminés par courriel. Des discussions ont été tenues par téléconférence entre les deux tours de questionnaires.
Résultats: Un modèle interdisciplinaire comportant des rôles différents et complémentaires pour les fournisseurs de soins de santé est ressorti du processus. D'autres éléments clés ont été mis en relief : dépistage des problèmes nutritionnels, promotion de la santé et prévention des maladies, collaboration, planification et évaluation, soutien administratif, accès aux soins et directives du médecin ou actes délégués. Selon le modèle proposé, la diététiste professionnelle est le membre de l’équipe responsable de la gestion de tous les aspects des services nutritionnels, de l’évaluation des besoins jusqu’à la mise en oeuvre des programmes et au soutien de tous les services nutritionnels.
Conclusions: Le modèle de services nutritionnels interdisciplinaires proposé mérite qu'on en évalue le coût, l'efficacité, l'applicabilité et la pérennité dans un milieu de soins de santé primaires dispensés par une équipe.
